Identify who created the record
Write down the responding agency, report number, officer name if known, crash date, exact location, and the names used in the report. A city police department, county sheriff, Oklahoma Highway Patrol, turnpike response, or another agency may keep different parts of the record.
- Use the agency name shown on the exchange sheet, citation, report, or insurance correspondence.
- Keep the report number and citation number exactly as written.
- If more than one agency responded, list each one and what it appeared to handle.
